Solution:
For some reason there was an old version of Phaser (1.3.2) installed
with CCP4 6.0.2, which actually comes with version 1.3.3.

I'll just paste Randy Reads' reply, which probably explains best the
case:

CCP4 version 6.0.2 is the one where the
quotes around filenames were introduced, and Phaser version 1.3.3 is
the first one that could handle them. So you need to make sure that
the correct version of Phaser is first in your path. As a workaround,
you could use the "Run&View Com File" option from ccp4i, and remove
the quotes from around any filenames, but that would quickly become
tedious! Alternatively, you could run Phaser from shell scripts.

Actually, I would really recommend upgrading to the latest version of
CCP4. In addition to all the improvements in other CCP4 programs, it
comes with version 2.1.4 of Phaser, which has new SAD phasing and MR
+SAD features that aren't present in 1.x versions. CCP4 has become
very easy to install.


We didn't upgrade CCP4 due to clashes with other programs used in our
group, but we installed the current Phaser and it runs fine now.
